Actually if you do a little research this is not that uncommon with this particular model. Seems it’s something to be aware of.

I had that happen with my 9mm SBR years ago (7" 9mm upper with CMMG bolt carrier on a registered AR-15 lower). I was shooting suppressed, then suddenly there was a super LOUD shot. It blew the side of the case out and jammed, and that was it.

It’s a straight blowback system, not entirely surprising. Most AR9’s have similar risks.
