I just got my package in the mail and am filling out the paperwork and would like to throw out a couple questions:
1) What do we do with the fingerprint cards? Do we do them ourselves, or do we have them done by LE?
2) I've lost the card from my class for a Mass LTC (took the class in the late 80s and haven't needed it to renew), but I have recently qualified in the military on both the M16 and M9 so a copy of my ID card, DD214 and copies of those classes should qualify as the necessary instruction, correct?
3) Finally, the white sheet that says to be signed and notarized - do I fill out the personal data there, or do I just sign it, and the data is filled out by the licensing agency? I'm asking because it looks like it winds up being the permit, and I don't want it to look like I've filled it in with crayon and have it sent back.
