Change of address notification

Asking this question for a friend.
He is moving from Newton, MA to West Roxbury, MA (Boston?).

As I recall, MA LTC holder has to notify in writing (cert. mail) local police chief, police chief for the new town, and the Executive Director of CHSB. (Correct?)

In case with West Roxbury, which is a part of City of Boston, who does he need to notify, West Roxbury Chief (they have their own PD) or the Boston Chief?

W Roxbury does NOT have their own chief! There is only 1 head of the Boston PD (superintendent IIRC) and that is who gets notified (at least to meet the law).
 
FYI: While there's nothing wrong with using the form from CHSB to report a change of address, there's absolutely nothing in the law that requires the change to be reported on any particular form, or to include photocopies of your license. If you want a short, simple form to use, try this one.

That's a good form, just one small suggestion. Why cut off the portion with the GOAL logo? (other than the instructions up there) I think the GOAL logo sends a message that we all (should) support.
