Since he is in NH, I doubt forms referencing MGLs would help much.

buickgn, why don't you just call whoever is responsible for issuing licenses in your city/county and ask? Ask if there is a form, or is a plain letter enough? And if so, to whom it should be addressed to.

This letter is very simple. Just say that you moved into the town and you have a NH Pistol Revolver permit. Nothing else need be said.
