Anyone ever see this hapoen to a shotshell?

These are factory loads, not reloads mind you. The first time it hapoened , it was somewhere around 6-8 rounds out of a box, and 5 other boxes were shot out of the same gun that day flawlessly. The following Sunday it happened 4 times out of a box and two other boxes were shot that day flawlessly. They are Remington Game Club #7.5 12 GA. The gun used was a Browning Sporting Clays Gold.
Remington was called and they are we ding out a box and shipping label to exam in them. Nice some shells are missing because someone the old timers wanted to look at them and I never got them back.

I have had it happen a few times but not recently, I shoot and reload Gun Clubs exclusively.
I work a batch of 200 hulls, reload them 6-7 times and then trash them.
