It sounds like there was just a bit of luck involved. The shot hit just below the right eye. Although a 25 yard head shot it not out of the possibility for a competent shooter, it is not probable that someone firing defensively, under pressure, at a moving target in a situation where body armor is not suspected would aim for the head.
Ii see this same thing all the time at matches when a shot on a distant, and fully exposed, target hits the upper B zone (formerly referred to as the "head") of a USPSA target. When that happens, it's always a bit of skill that kept the round in the general vicinity of the paper, and luck that kept the high off center shot on paper.
