Online Form 1?

I have requested a Form 1 from the ATF twice. Both times I did not receive it. I received some other form.

Is this the proper form?

If it is,
Will the ATF accept this version printed on standard printer paper?
What is an acceptable answer to 'i' State why you intend to make firearm.

I've always found that a phone call to a ATF office gets you what you want. For various forms in quantity, they can be ordered using their order form. Jack.

Be sure to print double sided. You automatically get rejected if it's not double sided.

I've used the online forms from titleII for probably my last 4-5 transfers, just easier to fill em out and print them up than to find a type writer, etc... as stated just make sure you print them double sided.
