Did you look on their site? Gun Owners' Action League of Massachusetts | GOAL.org I don't know if they have a downloadable one but Mike S. on here may be able to help you or you could contact GOAL directly and see if they'd send one out to you.

I've been a member for a long time personally, just need some paper applications, and the ability to direct others to paper applications. From the website it looks like you have to make an account and join online. I'll get in touch with the guys directly but I really wanted to see if anybody on here knew how to "become a GOAL member/supporter" without using a computer.

It's basically a just scan of the preprinted, 2-sided form. Enjoy!
