Can someone please supply me the link to the electronic FA10 form?

I remember seeing a nice PDF version on a past post, but can't find it.

Is a .pdf version acceptable for use? If so, it solves the problem of local PDs being out of the official multipart forms, and concerns about the new Web-based version.

Yes it is, Just fill out and print. I even called to check with CHSB before I sent the first one I used because it only printed letters and not the boxes, wasn't sure if it would scan. They said no problem.
