Wow. I hope he's alright. I'd grab those three rounds off the table and check them for powder. Or at least destroy them so nobody decides to try them out.

The unburned powder is most likely from a round in the magazine that got decapitated. There wouldn't be much if any "unburned" powder from a chambered round after a kaboom like that.So many problems could have been the issue, as stated above. Powder, quantity, brass quality......
Lots of unburned powder went everywhere and ignited after the kaboom. Arms were burnt a little. You can see burn marks on the carpet below the rifle.

Yep ok I'm up to speed. Didn't look at everything before I opened my fat f***ing mouthA squib would result in the bolt failing. None of the lugs on bolt or barrel are damaged in pics.
