Quick question about the NH non-res CCW form

Hi guys,

I have my form all filled out, but im confused as to whether or not I am supposed to check the boxes down the right side labeled "original" "renewal" Record Check" and "fees recieved"

It appears to meto be something that they should check off but im not sure. Can anyone help me?

I think you're right - it's for the staff to check off as they go.
I don't remember checking any of them when I applied. Maybe I checked off Original, but I don't remember.

So do you think it would be safe for me to just check off original? and send it in?

Yes, I think so. You def. don't need to check out the other ones...
