Is it just me or is OSHA like the police, they show up after the fact, take notes and punish people but don't do a lot of prevention? If the place had 54 violations, shouldn't OSHA have found these prior to the explosion and saved 2 lives? What a useless governmental agency, oh wait, most of them are useless.

OSHA is a government fundraiser, nothing more. I worked in a precious metals refinery during college and they'd show up once every two weeks, make a list of petty BS violations and give the plant manager a document listing them along with a fine.

The company considered it part of the cost of doing business.
