The ATF Form 7CR (5310.16) Revised May 2005 may be found here:

It is unclear to me whether this can be printed at home and submitted. A safer course is to request offically-printed forms here:

The forms took a week from request to delivery for me.

I mailed my C&R Application Jan 28th. The ATF charged me $30 on my Credit Card Feb 2nd.

That's pretty fast.

No idea when my C&R will be approved, but at least the ATF is opening their mail quickly.
