change of adress question, about the form its self

First. you are not required by statue to use the form. Letters (to all of the Author-i-tays) is sufficient.
Is your question about how to state the unit number of an apartment?
If so, just write whatever is on the rental/lease agreement.
A letter to the appropriate folks, with:
Name
License #
Old address (on the LTC)
New address :123 Fake St, apt (or unit or whatever) 4, Happytown, PRM.
If you think you have to use the form, just come as close as you can.
