BFS numbers for MA Instructors - Application?

Hi all, I just received my NRA Instructor creds in the mail, and now I need to apply for my BFS number, which is required by MA. I've been unable to find a copy of the application (or instructions) online, and I've also been unlucky in contacting the state police licensing or F.R.B.

Does anyone know where I can find a BFS number application? I'm very eager to start instructing, and i believe this is the last step. Thanks all!

Odd that your training counselor did not have one for you.....
