Replacement for lost LTC card

Can anyone tell me the proper procedure to request a replacement for a lost card? Is this done through my local PD?

Pursuant to 501 CMR 9.00, you are required to notify the Issuing Authority and DCJIS of the loss, prior to requesting a replacement.

Replacements are issued at no cost by your Issuing Authority.

I notified the clerk at my local PD. She got me a replacement within a week.
