? on FA10 for out of state rifle purchase

I did some searching and can't seem to find the answer to this.

I bought a rifle from a dealer in NH. It was modified to be MA compliant (no flash hider, pinned stock, and the bayo lug was ground off)

So I have to file an FA-10 for this to comply with the MA rules. In the upper right hand corner I cross off "registration" and I put all my info on the bottom.

Here's the question: I don't have the dealers license number information - for the top part of the form, do I need this? Or do I just fill in the dealer's basic info and the firearm info and file it with that?
 
I'm sure someone will come along and correct me if I am wrong, but I believe you are only doing a "registration" so no "seller" or "dealer" information is required.

I knew it was a registration, I just wasn't sure if I had to put any info into the form for the dealer who sold it to me.
 
Well, if you have the normal form with the instruction page, it specifies as such on the instruction page.
