There was a young man in Framingham who had a tattoo like that. An officer spotted it, and a subsequent search found an identical gun in his possession.

Bad day for the dude.
 
And I for one will not carry a 1911 in any other condition BUT "cocked and locked"!
 
I've carried them on duty before, always empty chamber, safety off, full mag. Easier to rack the slide than to flip the safety off for me, not to mention that it's Army policy in a peacetime situation, unless doing a particular kind of Guard Duty.

That is a malfunction and should be fixed, regardless of carry preferences.
 
My safety (aftermarket) doesn't like to engage. I've got a new Mil-Surp one, just haven't gotten around to installing it, and it's a non-issue, since I don't use that safety, but half cock or an empty chamber instead. But then, I only use my 1911A1 in the woods or at the range.
 
It was eventually, it just took me awhile to find someone that could do it. Besides that, the 1911 was my first carry gun, I was still somewhat ND shy. Haven't had an ND yet, however now I'm not so timid as to the point that I wont carry a live round in the pipe.

Skidding a little farther off topic here, Taurus PT111 millenium Pro was an excellent carry trainer. DAO with a manual safety cured me of my inhibitions rather quickly.
 
I'm not timid, in my line of work, an ND is a definite damper on your career, if not an end to it. LEO's get it easy compared to the military in this area. The Army only recognizes weapons malfunctions as AD's, and the gun still has to be pointed in a safe direction, or it's an ND.
