New FA10 questions

Ok so been going to surrounding PDs for FA10 forms response "you go online now and do it" I did some looking around about the E FA10 system all I found was info from GOAL for june 9th.

I cant seem to find the answer to a few questions.
1. Is CHSB still accepting FA10 paper form ( I see they removed the PDF format copies)
2. Will CHSB send residents paper FA10 forms ? I been tring to call but work gets in the way.
 
Do NOT use the online form. There are serious security issues, that have yet to be addressed. There is a printable form available on this website, but I can't seem to find it right now. Someone will chime in with the link. Yes, they are accepting the paper forms. I doubt they are mailing forms to anybody. Forget about finding one at a local PD. They are clueless.
 
Last I recall there was still some question as to whether they were accepting the non-numbered .pdf forms. I don't think we ever got any clarification.

Email with the State's PDF form and directions sent.
 
Last I recall there was still some question as to whether they were accepting the non-numbered .pdf forms. I don't think we ever got any clarification.

If I recall correctly, it was clarified in another thread, that they will accept the non numbered .pdf forms.

I did a transfer recently using the fillable form and sent it in. EOPS did not complain.
