Muzzle brake

Might be the way they discharge the gases, being one has more pressure could be extra set of holes or something in it. To be honest it could just be a way to sell the same product for mor money lol.
 
On jse surplus they have .223 and .556 muzzle brakes what's the dif ?

Must be installed with a metric adjustable wrench. These are very hard to find outside of metric countries like Europe.
