I don't think you have to notify florida, but CYA and notify MA, and at that point your MA permit will be null and void.

No forms required. Just send a letter with info (can be found off GOAL.org) to CHSB and town that issued LTC, Certified Mail Required.

That will cover you in case you ever have to get a MA LTC again.

Note that there's no legal requirement that you provide a copy of your LTC as the state form requests. It's simply a simple way for them to make sure that you got the license number correct. And of course to make you do just a little bit more than the law requires.

I heard that they are creating a new CMR to cover that new requirement.
