MA state certification

Does anybody have a link to the forms I need to get certified with the state? It only took them 5 weeks, but I finally got my NRA basic pistol cert today and now I can't find the state forms anywhere.
 
SR,

We need more info:

- Certified as an instructor with MSP? [Don't know, but Lynne or Ken will know.]
- Getting an LTC? [Forms are online from EOPS website and at PD.]
- Certified by Dept of Mental Health as a "What"? [lol]
 
MA Instructor

MA state instructor certification costs $50, and is a very simple process if you already have an NRA instructor's certificate.

Obtain the application by writing:

Massachusetts State Police
Firearms Licensing
470 Worcester Rd
Framingham, MA 01702
 
Rob is right. I thought that I had the application form in machine readable form, but can't find it or a link to it anywhere, so you'll most likely need to write them for it. Unless it's changed since they first implemented it, all you'll need is a copy of your NRA credentials and (most importantly) the $50.
