Duplicate E FA-10 form?

Hi all,

I did a transaction on my iPhone today, and when I hit save the transaction, that file popped up when I tried to save it. However, afterward, the web page "timed out" or "crashed" and I can't find the file on my iPhone.

My question is, if the file did get save, where would it be on my iPhone? I used the search function and I can't find anything.

Can I obtain a duplicate transaction form? I've done some forum searches and it looks like it was back in 2013 when people had issues with the system and there was a number to call?

you can email the frb and request a copy. They ask for date of sale, buyer/seller info etc...
