documenting your reloading

Does anyone have a spreadsheet already setup that they wouldn't mine sharing before I go an reinvent the wheel (which I'm know for)?

I'm thinking I'd document the following, am I missing anything here?
caliber (9mm)
brass (brand condition (new/once-fired etc)
primer (man/type)
bullet (shape, weight, manufacturer)
powder (brand and weight)
OAL
velocity/SD
accuracy (group size)
feeding issues
comments (smokey, dirty/clean, leading etc)

Did I leave anything out?
 
Attached is the format that I made up.

The processed chrono. data figures itself out based on your raw velocities that you enter. If you be sure to update the projectile weight, it will also give you your power factor for action pistol.
