Bolt carrier got hung up

Ok this is a brand new set up.I have a fail zero bcg and the palmetto premium upper with the fn barrel And geisele mlok rail.when I brought it to the range it fired twice then the bolt would not go into battery it was super tight. I will say when I check the head space it was a fine. I will post some other pics s of the bolt if anyone wants.

mash that forward assist. Sometimes a new build needs a break in.
Case has been ejected at this point- right?
 
Not knowing anything else:

Pull it apart and clean it. Maybe a metal filling got into the locking mechanism?

Not sure I'd 'force' it into battery. At least not yet. Do you have another bolt, another upper, swap parts and see what works with what.
 
I tried another bolt it worked fine

Strip and clean the bolt, is it me or are the forward assist serrations low on that bolt?
Look in the carrier Key for a a primer
 
I’m going to do a full inspection tomorrow I will let everyone know how it goes. Thanks for the help.
 
I played with one of those fail zero carrier / bolt combo's. Total trash. The coating started flaking off inside of 100 rounds.
 
Do you have a qd sling point on the back of your rifle where the buttstock goes?
Happens on one of mine it'll rub off after a few hundred rounds

Nope don’t have a sling on it

also I’m def dumping this bolt I should of just went with a bcm or spikes tactical bolt I would say never buy a fail zero bcg I feel strange just saying the name
 
Yes I inspected the item rifle and checked head space with my gauges
But did you clean everything , the rust and corrosion inhibitors they smother everything in becomes sludge, quickly.
Your firing pin hole looks awful for 2 rounds. Looks plugged up?
